  2. Required - Because of intl. credit card fraud it has become necessary to also require you fax a copy of either your Passport or some legal ID. This also protects you by insuring no one other than you is able to use your credit card.)
  3. AND - Also Print out this Service Authorization Form separately, sign it and fax it to : 1-425-702-8758

  4. ALONG WITH LEGAL IDENTIFICATION (example - faxed or scanned copy of passport) & faxed/scanned copy of front/back of credit card used to secure account.

For Callback Customers - Minimum Usage Requirement - Because of a finite number of available DID (telephone) lines, there is a $15 minimum monthly usage per telephone line you register to use with callback. This minimum usage requirement can be satisfied by totaling the customer’s aggregate usage on all of Telegroup’s services. For customers who cannot commit to that minimum, we have a service called Global Access VIP, which uses answered signal call and a PIN code.
